attempting to obtain approval from an insurance company for
a patient's medication. Modifier is doubled if the insurance
company phone menu is more than five layers deep.

applies to each 15 seconds of time you are on hold when
another doctor wants to speak with you and, instead of
placing the call personally, has his/her secretary call and
make you hold to wait for the calling physician to
come on the line. Modifier doubles if on-hold music is "The
Best of Cher."

minutes before casually admitting failure to either fill or
